Through daily practice, the PQ® Program helps identify and decrease negative thoughts (your Saboteurs) and works to increase positive thoughts (your Sage).

You get to build a foundation of mental fitness by strengthening three critical mental muscles to shift the balance of power from your inner Saboteurs (your negative self) to your inner Sage (your positive self).
Week 1 — Self-Command + PQ Reps
Theme: Build the habit under pressure (shifts, inbox, consults, family life).
• Create a realistic micro-rep plan (when/where on shift)
• Identify top triggers (notes, pages, conflict, admin burden)
• Build "pause protocols" for clinical settings (30–60 seconds)
Commitment: one daily anchor moment for reps
Week 2 — Judge
Theme: Stop practicing medicine as if you're constantly being graded.
• Identify Judge scripts (self/others/circumstances)
• Separate performance standards from self-worth
• Convert Judge language into neutral data + next step
Commitment: one Judge-to-Sage switch in a predictable trigger
Week 3 — Accomplice Saboteurs
Theme: Your strengths become liabilities under stress.
• Identify top accomplices (Controller, Hyper-Achiever, Pleaser, Avoider, Stickler, etc.)
• Map patterns to roles: leader, teammate, partner, parent
• Choose one accomplice to target (keep it narrow)
Commitment: one behavioral experiment (boundary, delegation, asking for help)
Week 4 — Empathize
Theme: Compassion without collapse; empathy for self as a clinical skill.
• Distinguish empathy from over-responsibility and rescuing
• Apply empathy to self-talk, colleague friction, patient/family tension
• Introduce repair: quick scripts after a Saboteur reaction
Commitment: one empathy-based repair or boundary conversation
Week 5 — Sage Perspective + Explore + Innovate
Theme: Curiosity is a performance advantage; innovation starts with reframing.
• Practice "What else could be true?" in conflict and leadership moments
• Explore alternative interpretations (delays, "noncompliance," friction)
• Innovate a new response: redesign one workflow, conversation, or boundary
Commitment: one Sage experiment that improves a real friction point
Week 6 — Activate
Theme: Convert insight into action—small, consistent moves that stick.
• Choose 1–2 high-leverage behaviors (sleep boundary, inbox boundary, delegation, meeting behavior)
• Build an activation plan: triggers, replacement behavior, accountability
• Weekly review: What worked? What didn't? What's next?
Commitment: one courageous action + one protected boundary
Week 7 — Navigate + Applications
Theme: Sustain under complexity; relapse-proof your operating system.
• Plan for future stress cycles (call weeks, family demands, crises)
• Create a Saboteur early-warning checklist + recovery protocol
• Integrate PQ into physician identity: leadership, communication, career direction
• Close-out: post measures, testimonials, and next-phase pathway
Week 8 — Navigate + Applications
Theme: Consolidate your 7-week journey:
Build your personalized mental fitness maintenance plan for high-stress clinical cycles
Create your "Relapse Prevention Protocol": early warning signs and recovery strategies
Translate PQ tools into everyday physician life: patient encounters, leadership moments, and family boundaries
Reflect on transformation: pre/post assessments, wins, and lessons learned
Design your next 90 days: sustaining PQ reps, accountability structures, and community support
You get to build a foundation of mental fitness by strengthening three critical mental muscles to shift the balance of power from your inner Saboteurs (your negative self) to your inner Sage (your positive self).
Self-Command Muscle
Week 1
Through a breakthrough process called PQ Reps, you get to boost your Self Command muscle. Each PQ Rep only takes 10 seconds and can be done with eyes open or closed. With each PQ Rep, you develop greater mastery over your own mind, quiet the negative, and activate the positive region of your brain.
Saboteur Interceptor Muscle
Week 2-3
You get to intercept your top Saboteurs the moment they try to hijack your thoughts, feelings, and actions. In preparation for this practice, you get to expose the lies, limiting beliefs and damage of your Saboteurs, so they can no longer fool you into thinking they are helpful to you.
Sage Muscle
Week 4-8

Aren't negative emotions sometimes helpful?
Some people believe that negative emotions are helpful. For example, they think stress gives them their performance edge, or beating themselves or others up is helpful for continual improvement. Feeling pain for a split second when your hand touches a hot stove is indeed useful, delivering an important alert. But your Saboteurs keep your hand on the hot stove, continuing to feel negative emotions which harm both performance and happiness.
